Real Health Medical is a Christ-centered, integrated medical wellness center partnering with clients through personalized health care and coaching from a functional medicine, root cause approach. Your protocols will include both clinical and lifestyle recommendations to restore health at the foundational level.

Take a moment to complete the New Client Intake form below that applies best to you, and you will receive an email with more information on scheduling! Once your Intake Paperwork is received, our Intake Coordinator will reach out to you via email within 2-3 business days with initial appointment recommendations and pricing estimations based on your goals. NOTE: Real Health Medical is a cash pay practice, we do not bill insurance directly for services nor provide CPT codes or submit pre-authorizations requested by insurance companies for uncovered Rx medications or hospital procedures.

**However, labs can be billable to insurance depending on your plan. Please contact your insurance or share plan to ensure lab costs will be covered and which lab vendor will be approved to process.**In house phlebotomist for Roswell Location works with Quest Diagnostics.**In house phlebotomist for Stockbridge Location works with LabCorp.

NOTE: We may also require a deposit as part of New Patient Registration at the time of booking your initial visits to secure your appointment slots. Any deposit collected will be applied to your bill at checkout.

New Client FAQs

  • How do I become a new patient?

    Real Health Medical is currently accepting new patients. To become a new patient with Real Health Medical complete our Intake Form on our Get Started Page and our Intake Coordinator will reach out to you via email with details about your initial appointment.You will need to email her back and let her know if you’re ready to move forward and get scheduled.

    Please allow up to 2 days to hear back from the Intake Coordinator via email about your appointment. If for some reason her response does not show up in your inbox, be sure to check your spam and junk folder.

  • How long will I have to wait for an appointment?

    New client appointments cannot be scheduled the same day, but typically there is an opening within 1-2 weeks with a practitioner.

  • How much does it cost to be a patient at Real Health Medical?

    Clinical appointments are cash pay based on practitioner time. Treatments are priced a-la-carte as well as discounted pre-pay packages with the exception of our Real Health Kids Pediatric Membership. As a New Client, your Intake Coordinator will provide an estimate of initial visit appointments. Every treatment protocol is unique to each client, so costs vary greatly depending on what is recommended (from a few supplements, to a full collection of therapies over 8-12 weeks).

  • What are your hours and days of operation?

    Monday - Thursday 10-5
    By appointment on Fridays

  • Do you accept insurance?

    We are a cash pay practice – labs can be filed with your insurance for potential coverage depending on your plan – We recommend checking with your insurance first to verify if labs will be covered. HSA (Health Savings Account) and FSA (Flex Spending Account) can be used as form of payment. Credit Cards incur a convenience fee. We want to be able to have the freedom to provide the care our clients need without restriction of insurance limited coverage. We also provide discounted packages when multiple treatments are recommended and spread payments out over a reasonable timeframe if needed.

  • Can I receive an itemized statement to file with my insurance or sharing program?

    Yes, our Billing Department can provide an itemized invoice of all services provided, the date of services and charges for those services. Since we do not bill insurance directly medical codes will not be provided.

  • Do you provide emergency/urgent care services?

    On a limited basis we can provide urgent care to currently active clients.

  • What is your cancellation policy?

    Clients who cancel outside the 48 hr window may incur up to $50 fee of their visit cost. Same day cancellation or no show may incur the full charge of the visit.
